Doctrinal Agreement:

I have read, understand, and agree with the Statement of Faith of MissionWorks and MissionConnexion. I will work in agreement with and lead according to those statements.

I understand that MissionConnexion requires information from me and they may choose to request personal references or contact my pastor in evaluation of my qualifications to serve. I authorize and release personal information to MissionConnexion to make those contacts, if necessary.
